We file and win No Surprises Act disputes for out-of-network providers

Flat fee per arbitration. You keep every dollar of the award.

Payers count on out-of-network groups never pushing back. The No Surprises Act gave providers a federal arbitration process, called independent dispute resolution (IDR), and the published record shows providers win it 4 out of 5 times. Lido IDR runs that process for you: eligibility, deadlines, evidence, and filing.

What is Lido IDR? Lido IDR files No Surprises Act arbitration cases (federal IDR) for out-of-network providers and the billing companies that serve them. We find the eligible claims, track every deadline, prepare the evidence, and submit the filings, for a flat fee per arbitration instead of a percentage of the recovery. The service is built by Lido, a software company, on top of every federal IDR decision CMS has published.

Why groups come to us

Deadlines are short and easy to miss

Every No Surprises Act deadline is counted in business days: 30 to start negotiation, 30 to negotiate, 4 to file for arbitration. Miss one and the claim is gone. Some payers ask for 45-day extensions hoping you run out of time. Most groups track all of this in a spreadsheet. We track every deadline on every claim.

Paid is not the same as paid fairly

Underpaid claims are invisible. A claim billed at $554 and paid at $74.74 never appears in a denial queue, because the payer technically paid it. Groups accept those scraps for years because contesting each one by hand costs more than the difference. Arbitration changes that math: the median federal IDR award runs 4.7 times the qualifying payment amount the payer started from.

You did not sign up to run an arbitration desk

Portal re-keying, evidence packets, offer deadlines, fee receipts. Groups that file at volume do most of it by hand, and practice leaders tell us they do not want to be in the arbitration business. We take that work. You approve the strategy, we run the arbitration.

Most filers take 15 to 25 percent of the recovery

IDR vendors and law firms usually charge a percentage of what they win for you, the dollars recovered above the payer's initial payment. On a $50,000 recovery, 15 to 25 percent is $7,500 to $12,500. The work is the same either way, so we price it the same either way: a flat fee per arbitration, and the award is entirely yours. How the fees compare.

How it works

Send us your remits. Software finds the eligible claims, tracks every deadline, and prepares each filing. Anything that needs judgment goes to a person, and no offer goes out without your approval. We run the whole dispute, from first notice to the arbiter's decision to the award hitting your account.

Swipe to see the full pipeline

  1. We find the eligible claims. From your remits, we flag which out-of-network claims qualify under the No Surprises Act and what similar claims have won in your state and specialty.
  2. We prepare and file everything on time. Open negotiation notices, IDR initiation, offers, evidence packets, each inside its deadline. You see the status of every dispute.
  3. You keep the award. Payers pay your group directly. Our flat fee per arbitration is the same whether the award is $500 or $50,000, so our incentive is simple: file everything worth filing, and win it.

Built on all 3.39 million federal IDR decisions

Lido IDR is built by Lido, the document automation company. We started by publishing the entire federal IDR record at idrexplorer.com: every dispute, every payer, every IDR entity, and award benchmarks for more than 800 specialty and state combinations. That data now powers our filings, from picking winnable claims to setting offer amounts. More about us.

Common questions

What does Lido IDR cost?

A flat fee per arbitration, quoted for your claim volume on a 30 minute call. We never take a percentage of your awards. Separately, CMS charges each party a $15 administrative fee per dispute, and the certified IDR entity charges a fee that is refunded to you when you win. Full fee breakdown.

Do you handle the arbitration itself?

Yes. Federal IDR is decided on written submissions, there is no hearing. We prepare the offer and evidence, submit them, and track the case through the arbiter's decision and the payer's payment. The one thing we do not do is litigation: if a payer ignores a binding award, we document it, file the CMS complaint, and hand the record to your attorney.

Who can use federal IDR under the No Surprises Act?

Out-of-network providers, for emergency services, for non-emergency services at in-network facilities such as hospitals and surgery centers, and for air ambulance transport. In states with their own surprise billing process, claims under state-regulated plans go through that process instead. State or federal, explained.

Do you take a percentage of awards?

No. Awards are paid by the payer directly to your group, and you keep all of them. Most IDR vendors and law firms instead take roughly 15 to 25 percent of the recovery, the dollars the dispute wins above the payer's initial payment. What that costs on your recoveries.

How often do providers win?

Providers won 85.8% of decided federal IDR disputes from 2023 through 2025, with a median award of 4.7 times the qualifying payment amount and a median of 37 days from filing to decision. Computed from the CMS federal IDR public use files at the dispute level.
